New Patient Information

Because the office of Dr. Bishop specializes in endodontic care, most of our patients come to us in discomfort. We do our utmost to make patients more comfortable as quickly as possible. Whether you come to us as a referral or an emergency, we will start by carefully examining your area of concern. Then, we will spend time consulting with you on your current condition and treatment options. In some cases, we are able to begin treatment on the same day. For patients who suffer from complicated health conditions, or take a number of medications, we may need to complete a more thorough evaluation and begin treatment at a second visit. Please let us know right away if you have any conditions that may be of particular concern including diabetes, high blood pressure, rheumatic fever, artificial joints or heart valves, etc. If you require pre-medication with antibiotics for any existing conditions prior to dental treatment please contact our office prior to your visit.  Bring the following documents with you to your appointment:

To expedite the consultation and treatment process, we invite you to complete our patient information forms prior to your visit. These documents give us the opportunity to offer you more personalized treatment from the moment you arrive. Please share your e-mail address with our team when you schedule your appointment. We will send you information about your upcoming appointment along with a link to click so that you may fill your information out at your convenience prior to your appointment.

We Assist You with Dental Insurance

Our team members file all insurance claims as a courtesy to our patients.  This does not however, transfer the responsibility of your financial obligation to the insurance company.  We are currently in network with many of the Aetna plans.  Your dental benefits are based on the agreement your employer negotiated with your insurance carrier. The patient payment portion covered from procedures varies depending on the coverage provided by your employer. Please take a moment to review your insurance benefit information to better understand the benefits your plan provides.  An estimate of the amount covered by your insurance company will be provided at the time of your treatment, based on the information they provide to us.  The estimate is never a guarantee of benefits or the amount that will be paid.  If the amount paid by the insurance company is less than or greater that the estimate, then you will be billed the difference or issued a credit in the event of an over payment.  Please let us know if you have questions about our financial policies or financing options prior to your treatment.
